The launch guide reports level 5, −40 reputation, and Wall Climb for the quest. Check your actual reputation total rather than assuming a fixed number of actions will always produce the same result.

Reported Muzan search areas are Mistfall Harbor near Ginzo, Iceveil Valley near the clothing shop, and the lower wooden paths in Hidden Mist Village. He can move. These are search areas, not guaranteed spawn pins.

The reported route asks for nine Spider Lilies and the capture and delivery of Dr. Higoshima. Look for lilies at night; follow the quest marker for the doctor. Complete the handoff and use the awarded Muzan’s Blood to transform.
